Beginning July 2, 2007 Palmetto Family Medicine
will have several new policy and procedure
changes.
- No NEW medicines will be prescribed over the
phone. If you need a new medication you must
call the office and schedule an appointment.
- Please allow 24 hours for all refills to be
processed.
- Please bring your medications with you at
the time of your visit so your physician may
address any questions you may have and provide
refills if you need them.
- If you need a prior authorization you will
need to schedule an appointment to meet with
your physician. If your visit is for a prior
authorization please bring your drug formulary
with you to your visit. Please call your
insurance company and have them send you a copy
of your drug formulary before you come in to see
your doctor.
- Please bring your insurance card with you to
EVERY visit.
- Please come prepared to pay your co-pay at
every visit.
We appreciate the opportunity to provide your
family's healthcare
needs. These policies are intended to allow us to provide health care services
to as many patients as possible in a timely manner. Thank you for understanding
and cooperating.
Voice Mail
We have voice mail for your convenience. Leaving a
message prevents being on hold during peak hours.
Insurance Cards
Please bring your insurance card with you to every
visit. If you have a new card, have changed
carriers, or if any other changes have occurred with
your insurance company,
please notify our front desk upon arrival. We can
only bill your insurance
company for your visit if we have accurate insurance
information. If we do not have updated
information, you will be billed as a self-pay
patient and will be charged for the entire amount of
your visit. If your
insurance requires a co-pay please be prepared to
pay at the beginning of
each visit. If you do not pay your co-pay at the
time of your
visit we will charge you a $15.00 billing fee.

Payment for services
Payment for services rendered is expected at the time of service, including co-pays,
deductible and co-insurance. We accept cash, credit cards, or checks.

If you are unable to keep your appointment, please
call our office
the day before to cancel or reschedule. We charge a $25
no-show fee for any appointment not
cancelled 24 hours in advance. |

We do our best to see you in a timely manner when you come in for your
appointment. Therefore, if you are more than 15 minutes late, we reserve
the right to reschedule you for another time. |
Prescription Refills
Contact your pharmacy and they will fax
your doctor a refill request form.
Referrals
and Authorizations
If an authorization or pre-certification
is required by your insurance, we will need a minimum of
72 hours to contact your insurance company for authorization.
And in some cases it is necessary for you to be seen by our
physician first.
Labs & Your Insurance
Our
staff will attempt to determine the lab or facility that
your insurance covers, but it is ultimately your responsibility
as the patient/insured to confirm the correct lab or facility.
